Metal pieces and loose bolt found indside starter assembly

So I went to start my car this morning and was greeted with the sound of a bad starter. The car ran absolutely fine the night before, no CEL and no strange noises. Since this is the third starter I've replaced in six months the whole thing felt suspicious. I had the car towed to my mechanic and he took out the start and a piece of metal along with a loose bolt fell onto the ground. The pieces completely stripped the starter gear. I've attached pictures of the pieces that fell out and starter gear. After using a magnet more metal pieces were found. My question to you guys is has anyone heard of this happening before? And specifically does anyone know where these pieces may have come from? My mechanic has been unable to find anything broken without dropping the transmission and taking apart the clutch, which will cost me $700 not including the cost to repair whatever has broken. Car has 150K miles.
