Loss of Boost Pressure while Driving on the Highway - Help Please

I have recently been experiencing loss of boost pressure when I drive on the highway for periods longer than an hour or so. Normally I get around 18-19 psi peek with a small drop down to 16-17 psi but now I will only get between 4-6 psi max in any gear. This has happened a few times with the first time happening while I was down at the Tail of the Dragon back in November while driving on the Cherahola Skyway. Once I turn the car off and let it sit for a few minutes everything is back to normal so I'm really lost at what to do next.

The last time this occurred it happened on my 30 min commute to work, so it seems like it is starting to happen more often on shorter drives. When it happened the last time I got a CEL which I scanned when I got home from work. The 3 codes I got were:

I'm wondering if it's the PCV or a vacuum leak somewhere. Any help on what to check/look out would be greatly appreciated. Thanks so much!

Generally the lean running would be a vaccum leak. Which diverter valve do you currently have?
