    IDOT:

How do I report a pothole on a state-owned highway?
The Illinois Department of Transportation is responsible for the repair of potholes on all state maintained highways. Motorists may report pot holes on state highways by calling 1-800-452-4368 or by using the Contact Us Function of this webpage.

Re: How to file a Pot Hole claim with the City of Chicago

wow this is nice to know.... i bent two of my rims during my stay here in the chicago area already. However im not sure if its on the illinois side or the indiana side... Does this matter if its potholes only or does it include elevations on the road. (2 inch step in the middle of the road ~55mph)
