Contact aixgelo78, RAD69, mat@induktion, or ckymike on AW. All have done this. I believe there are three or four others as well. My feeling though, is that unless you are prepared to do the work yourself, it's going to be quite expensive. There's more to it than just physically swapping a motor and transmission. It may be that given the falling prices of B5 S4s, that there is no advantage to such a swap, especially since when you're done, you still won't have a complete S4. At a minimum, you'll need motor, and most accessories, subframe, ECU, axles, knuckles, wiring harnesses, transmission, driveshafts, rear differential, and full exhaust. There are several exterior and interior differences as well, that you would need for a truely complete conversion.

Is your A4 a 2.8, or a 1.8T? Quattro? DBW? All are factors to consider when evaluating this swap. You won't be the first, and I find it highly unlikely that you'll be done by Waterfest.
