There are threaded holes in the hub, and you simple screw your lug bolts into them. The spacer will simply sit between your wheel and your hub. Your hub and oem wheels should be hubcentric, meaning there will be a small lip to set/center the wheel on, but it doesn't hold the wheels there perfectly without lug bolts.
1. Take off your wheel
2. (optional) Apply some antiseize to the back of the spacer if you have some and want to. This will help with the uninstalling of your spacers, and they will get stuck on the hub due to weather and elements.
3. Line up the correct holes on the spacer, and push the spacer against the hub as good as you can by hand
4. With rubber mallet (soft so it doesn't harm the alloy spacer) hit the spacer around the center to seat it. When it's on there good, hit the outside edges a little harder (in kind of a star pattern) to make sure spacer is all the way flush against the hub.
If the spacer is not on there all the way, it will probably cause vibration issues.
5. Set your wheel on there (I like to lower the car so the hub is level with the wheel so I don't have to lift it up) and thread in your new lug bolts by hand. Any order is fine since you're just threading them in partially.

With the wheel in the air begin to torque your lug bolts down
in a star pattern to
88ft/lbs a piece with your torque wrench. If the weight of the car is on the wheel, it might not be perfectly centered. Same for if you tighten in a circle pattern and not a star. This can also cause vibration issues if you do it wrong.
7. Lower the car back down, and check each lug again with your torque wrench just to be sure.
8. Re-check your lug bolts after you drive around for a day or two.
